If you have had more than 2 evaluations filed on your insurance, you will be responsible to pay in full on the day of your appointment.

Please bring an insurance card or your insurance information if you would like us to file your insurance for you.

We are in network with Delta Dental Insurance and Cigna DPPO. We are not in network with Cigna Advantage or Cigna Total. Please call your insurance company if you have any questions regarding in network and out of network benefits.
